  • With One Path Pilot, your practice can seamlessly enroll new RPM patients through HCUnity’s system. After completing the billing process with CMS, do you partner fully with HCUnity or not at all?
  • Choosing to partner with HCU means you agree to transition all your patients to HCUnity within a couple of months. HCUnity can help you with the transition as well.
  • If you choose not to continue, you must pay for the months you used the tablet/app and for the collection of data sent to your office.  You will also be responsible for getting your patients to return the equipment, which includes the HCUnity Hub Tablet and app exclusively designed for HCUnity RPM.
  • Rest assured, if the tablet is not returned, it will be rendered completely unusable.

 

  • We provide each client with an RPM kit identical to what patients receive.
  • This kit is used to train your staff, who will then train new RPM enrollees right after they are enrolled, The whole process onboarding to training might take 20 minutes.
  • Patients not ideal for RPM can be transitioned seamlessly into Chronic Care Management (CCM) if they are suitable for the service.
